Witkowski died in Homberg, Germany, from non-combat related injuries sustained on April 30, 2004 in Kandahar, Afghanistan. He was assigned to 7th Field Artillery, 25th Infantry Division (Light), Schofield Barracks, Honolulu, Hawaii. Died on May 1, 2004.

"Well there so much i can say about my good friend. I remember the first time i met Phil i was throwing the football around with my son in the backyard, and he came over to say hello and offered me a beer, from that point on it was on. We BBQ together, played spades while our wives were cooking in the kitchen. We would have so much fun. He never said or talked badly about anyone.
