Ayala died in Ghazni, Afghanistan, when the CH-47 helicopter he was on crashed. He was assigned to the 5th Battalion, 159th Aviation Regiment, 12th Aviation Brigade, Giebelstadt, Germany. Died on April 6, 2005.

"I served with Dave in 601st ASB. I just recently heard what happened to him as I lost contact with a lot of the men and women I served with. God has never called home a finer soldier than Dave, or a better person. Dave could always find something to make you smile about no matter how bad a situation you were in. He would always get pumped when he talked about becoming a Warrant Officer. My condolences to his family."
